No matter how healthy you are, having a primary care doctor is a good idea. Primary care providers act as the quarterback for all of your healthcare needs and are the first line of offense when it comes to preventative medicine.
Our office is closed daily for lunch from 1:00 pm-1:45 pm. However, you can still submit a question to our office via our contact form or email during this time, and we’ll answer as quickly as possible.
We follow the CDC guidelines as it relates to COVID-19. In addition, we require face covering while in the office, social distancing, and hand hygiene which all must be strictly observed.
While we plan to accept all major insurance companies, we may be out of network with your plan at the moment as we are still going through the participation process with many payers. Therefore, it’s best to contact our office to see how we can help.
You will be emailed patient registration forms, which should be completed before arrival. Be sure to arrive early to give yourself plenty of time to complete the initial intake process. You can also bring any relevant lab reports, medication, and treatment plans you’re currently under to discuss.
If you have a deductible or co-pay, payment is due at the time of service. Your insurance benefits to confirm the amount required. We accept cash, checks, debit, or credit cards.
We request that you cancel your appointment 48 hours before your scheduled time to allow someone else to take your slot. If you do not cancel and fail to attend an appointment, you will be charged your co-pay amount for the missed schedule.
As a rule, new patient appointments typically take around 1 hour, with follow-up visits often taking less time depending on what you are being seen for.
AHF Wellness Centers and our affiliates offer free STD/HIV testing. NO INSURANCE OR PAYMENT IS NEEDED.
Your screening may include tests for gonorrhea, syphilis, chlamydia, and HIV. A certified testing counselor can take a urine sample, throat swab, and/or rectal swab to test for gonorrhea and chlamydia. Blood can be drawn to test for syphilis or HIV. HIV tests may also be done with a simple finger-stick test or a mouth swab. A certified counselor will walk you through the process.
We only gather the minimum information necessary and required to comply with regulations and to assist us in designing the best health programs for the people we serve. Therefore, we strive to keep the process as simple as possible.
